Once completed, all signed documents can be submitted via email to firstname.lastname@example.org or in-person. If submitting via email, please contact us by telephone to arrange an appointment to finish the processing. You will need a copy of your current Food Manager's Certificate, payment in the amount of $300.00 for the deposit (cash, check or credit card) and insurance certificate. Once processing is complete, you will be given access to our scheduling calendar and receive and access card for entry to the kitchen.
